Accusations are swirling that hospital-grade morphine—vital pain relief for critically ill patients—has been vanishing from the ambulances, and Jacob is the prime suspect. He swears he’s innocent, but everyone seems to be looking at him differently now.

Faith, a nurse and Jacob’s friend, is caught in the middle. She discovers that her partner Iain secretly went to Jan behind her back, effectively throwing Jacob under the bus without any proof. Faith is torn between loyalty to Jacob and anger at Iain, especially as she realizes she had no idea what was happening behind the scenes. Meanwhile, the ED is overflowing with patients, and tension is running high as they struggle to control someone’s pain despite large doses of morphine.

That’s when the nightmare really begins.

The morphine Jacob allegedly stole appears to be in the hospital system—ampoules are there, labeled correctly, but the medication isn’t working. Faith administers a dose to a critically injured patient, Fraser, but the pain doesn’t subside at all. Moments later, she administers morphine from the hospital’s ED stockpile instead of the ambulance supply—and this time it works instantly. The realization hits them like a thunderbolt: the problem isn’t Jacob, it’s the drug itself. Someone has been tampering with the morphine on the ambulances.

Iain tries to defend himself, but his guilt is written all over his face. He panicked, went to Jan with suspicions about Jacob, and now realizes he might have ruined an innocent man’s career. But the damage has already been done. Jacob, furious and humiliated, lashes out at Iain for doubting him. In one heated exchange, Jacob reminds him what it feels like to be judged by everyone, unsure if your entire career could vanish overnight—all because of someone else’s lies.

Faith watches helplessly as the two men she cares about tear into each other. Her own conscience is eating at her, because she knows she could have stepped in sooner to defend Jacob but didn’t. Meanwhile, the hospital continues to treat patients with potentially compromised medication. Every moment counts, and one mistake could cost a life.

The tension between Faith and Iain escalates when he accuses her of protecting Jacob while betraying him. She fires back that she had no choice—she was trying to protect her own job too, just as he had once gone to their superior, Max, to report her. If it shows two lines, then the ampoules are fake, confirming someone has been tampering with the hospital’s supply. Everyone gathers around the test strip, hearts pounding as they wait for the result.

In the background, Faith’s mind races. If the morphine really is fake, who would do such a thing? Why risk patients’ lives? Is it greed, a black-market operation, or an attempt to sabotage the hospital itself? And if the drug is real, what’s causing the strange reactions in patients—some unknown contaminant, or an undetected tolerance issue?

As the test develops, the tension among the three of them boils over. Jacob accuses Iain of hiding behind his badge while ruining someone else’s name. Iain accuses Faith of choosing Jacob over him, of loving one man enough to protect him but sacrificing the other. Faith breaks down, torn between loyalty, love, and survival.

Finally, the test result appears. Two lines. The morphine in the ampoule isn’t morphine at all. Someone has been systematically replacing the real drug with a fake substance, keeping the packaging identical so no one notices until it’s too late. That means Jacob is innocent—and someone much more dangerous is at work inside the hospital system.

But this revelation comes too late to undo the damage already done. Jacob’s reputation is in tatters, Faith’s relationship with Iain is broken, and the hospital is left scrambling to contain the crisis. Whoever’s behind the tampering is still out there, and the clock is ticking before another patient suffers—or dies.

The movie ends its act with Jacob walking away from the break room, his eyes blazing with a mix of vindication and rage. Faith watches him go, knowing she’s lost his trust forever. Iain stands alone, realizing that the real enemy isn’t Jacob, but someone far closer than any of them imagined.
